Release of liability & consent

Handling animals and other volunteer activities carry some risk of injury. By submitting, you acknowledge that risk and release HSLC (and its volunteers, employees, and board) from liability arising from your participation, and you allow HSLC to seek emergency medical treatment on your behalf if needed. You also agree to keep confidential information private.

Consent

I authorize HSLC to use photographs of me for public-relations purposes. For everyone's safety, I agree to be visually and audio recorded while volunteering.

Agreement

I have read and agree to the volunteer release-of-liability waiver, and the information I've provided is accurate.

We'll finish the signed waiver (and a parent/guardian signature, if you're under 18) at your volunteer training.
